We welcome your online registration for the Dementia Australia National Symposium 2020. 

It is important to read all the information on the Symposium website before you commence. Please note all prices quoted are in Australian dollars and are inclusive of goods and services tax (GST).

Instructions for online registration

  • Please ensure you have read and understood the cancellation policy on registration and accommodation options as provided on the website.
  • All information should be typed in upper and lower case (eg John Smith)
  • Please use the 'Tab' key to move from one field to the next
  • As you complete the details on each page, click 'Next' to proceed to the next section
  • You will have the opportunity to review and edit any registration details before the final stage of confirmation and payment
  • Google Chrome and Microsoft Edge are the preferred platforms for use of the forms

Registration payment

  • Online registrations can be paid by credit card (Visa/MasterCard and American Express) or payment via electronic funds transfer
  • Please note if a registration is made less than 30 days prior to the event (24 February 2020), credit card is the only payment option available

As part of the registration process we will collect personal information about you. For information on the privacy policy, please click here. If you do not give consent for your details to be used you cannot register for the Symposium and you will be redirected to the Symposium website.

Accommodation
A range of accommodation options are available for your selection within close proximity to the Symposium venue. More information about accommodation options can be found on the website.

Please note:
  • A valid credit card number must be provided to secure your booking. The card details will be forwarded to the hotel as a guarantee of your booking 
  • You will be required to settle your room and incidental expenses direct with the hotel on departure   
  • If you wish to pre pay your accommodation in advance by credit card or EFT upon invoice, please note this in the 'Special Requirements' section on the next page when completing your accommodation booking 
  • Accommodation is held from Monday 23 March to Wednesday 25 March 2020. If you require dates outside of this period, please note your preferred dates in the 'Special Requirements' section. The Delegate Services Team will contact the hotel direct and advise you of availability

Deadlines
The hotels impose strict release dates of 30 days prior to arrival, so any bookings made after Friday 21 February 2020 will be subject to availability.

Change of Booking 
To make a subsequent change or to cancel your booking, notification must be made via email to the Symposium office. Verbal changes or cancellations cannot be accepted nor will the hotels accept any changes direct.

Terms and conditions

Registration
Cancellations of more than 5 business days’ notice prior to commencement day of the first session, registrants may:
  • Nominate a substitute participant; or
  • Request a full refund
Cancellations of 5 or less business days’ notice prior to commencement day of the first session, registrants may:
  • Nominate a substitute participant; or
  • Accumulate 50% of refund
  • Provide a medical certificate covering the date of training to receive 100% refund 
No shows – registrants may:
  • Provide a medical certificate covering the date of training to receive 100% refund
  • Full fee is payable – no refunds   
Accommodation:
For accommodation cancellation policies, please refer to the individual hotel policies noted on the website. Any amendments to original hotel bookings must be forwarded via email to the Dementia Australia National Symposium 2020 Office. Verbal changes or cancellations cannot be accepted nor will the hotels accept any changes direct.
